New Patients

We are welcoming new patient registrations but please check that your address falls within our practice area. We cover parts of BD2, BD10 and BD18, but not all.

We need to have your NHS number to register you. You should bring your medical card with you if you have it,(if not you can check with your previous surgery who will be able to tell you) and some other form of identity such as a utility bill. We also need to see photgraphic ID.

You will be asked to complete a Registration Form and a New Patient Questionnaire. When you return the forms to reception we will check your ID and address verification . We will then arrange an appointment with the nurse for a New Patient Health Check and once you have had that appointment you will be registered at this practice.

You can search for your postcode by clicking the magnifier in the top right corner of the map, this will indicate whether you live within the boundary or not.
